return we expect the following commitment from our patients,
that you: |

Keep appointments or should it be necessary to
change an appointment, we require at least 24
hours notice. We do make a nominal charge for
repeated late cancellations and missed appointments,
but it is never our wish to do so |

|
Inform
us of any medical conditions or any changes in
your health that may affect your dental treatment,
as your health and comfort is our primary concern |
|
|
Speak
to the Dentist, Practice Manager, or an appropriate
member of staff if any problem should arise as
soon as possible, so that we may have the opportunity
to resolve the matter quickly. |

| Our
financial policies are as follows: |

Treatment
plans are agreed with all patients |

We
will explain the costs involved before treatment
begins and provide written estimates on request |

|
Should
it become necessary to change the treatment plan
such that additional charges or credits are applicable,
this will be explained and agreed with our patients
before treatment is continued |

We
request that all fees are settled at the time
of treatment. |
