Refer a Patient
We take referrals for orthodontics and will soon be taking referrals for endodontics and implants. Please fill in the below proforma, and send any relevant radiographs to: enquire@bedaledentalpractice.co.uk.
Alternatively click here to download a PDF referral form which can be completed digitally and emailed to the above address, or printed and mailed to our practice.
Please note that all referral treatment is on a private basis.
